It is our philosophy to care for you with respect, compassion, empathy, and diligence and are committed to excellent clinical care. We provide you knowledgeable and highly skilled expertise with innovative medical technology and an advanced EHR (Electronic Health Record) system in a comfortable, beautiful, and relaxing spa-like setting. We believe our relationship is a partnership and it is our goal to help you be educated, engaged, and empowered about your health. We feel every woman is special and deserves individualized care tailored to her specific needs . It is our goal to provide personalized , compassionate, and quality healthcare.
Our office hours are Monday-Friday 8am-5pm. Dr. Jerath is available for patient care appointments Monday-Thursday. Other appointments such as blood draws, lab tests, or NSTs (Fetal NonStress Tests) may be scheduled with our clinical coordinator (medical assistant) any day of the week, including Fridays. We are accepting new patients and welcome the opportunity to care for you.
During office hours, our phones are on and available 8:15am-11:45am and 1:15pm-4:45pm to personally address your healthcare needs. During our phone hours, we also have an automated system allowing you to access direct personnel as needed. Any questions or call that do not need immediate assistance are more efficiently addressed by sending a message through our secure patient portal.
Outside of our phone times, we have an answering service that can reach our physician 24/7 for any immediate or urgent needs.
You may request or schedule an appointment by sending a message through our secure patient portal, calling us directly during office hours, filling out our contact us form, or by sending us an email to email@example.com. Please do not send any private protected health information (PHI) through regular email as it is not secure.
If you are unable to keep your appointment, please call to reschedule as soon as possible, and kindly give us at least 24 hours notice so that we may accommodate the scheduling and healthcare needs of another patient as we often have a waiting list of patients who would like to be seen sooner. We understand unexpected scheduling conflicts and emergencies occur that cannot be helped and we will work with you under these circumstances to get you rescheduled in a timely manner. However, any appointments rescheduled with less than 24 hours notice, may incur a $25 charge on your account which may be posted against any credit balance on your account or be billed to your credit card on file. If the charge remains unpaid, you will receive a statement in the mail, payable upon receipt. This charge cannot be billed to your insurance as it is patient responsibility.
If you are running late for your appointment, please kindly call to inform us. We will do our best to accommodate you upon arrival, but you may need to wait to see the provider based upon availability. If you arrive more than 15 minutes late for your appointment, we may need to reschedule your appointment in order to respect the time of our other patients scheduled.
Cancellation and No Shows
If you need to cancel your appointment, please call to cancel or reschedule as soon as possible, and kindly give us at least 24 hours notice so that we may accommodate the scheduling and healthcare needs of another patient as we often have a waiting list of patients who would like to be seen sooner. Any appointments cancelled with less than 24 hours notice or appointment no shows, incur a $50 charge on your account which may be posted against any credit balance on your account or be billed to your credit card on file. If the charge remains unpaid, you will receive a statement in the mail, payable upon receipt. This charge cannot be billed to your insurance as it is patient responsibility.
Secure Patient Portal
We offer the latest in technology to ensure it is easy to interact with us. The patient portal enables direct contact with our doctor and staff and allows you to:
- Request an appointment
- Fill out health forms privately
- Ask questions of the staff or doctor
- Send emails to us 24/7
- Have access to your medical records and clinical summary
- Request prescription refills
- Pay your bill online
- Receive lab results, practice updates, instructions, and appointment reminders
The patient portal is the best way to contact us for any non-urgent questions or concerns that do not need immediate assistance. The portal is checked frequently and we respond as quickly as possible, but do ask that you allow 3 business days for a response. You may also reach Dr. Jerath directly via the portal. She reviews these messages daily and responds personally whenever possible. Our patients appreciate this privilege and find the portal to be extremely beneficial.
We will also use the patient portal to inform you of normal lab results and provide lab results or specific medical records as needed.
After Hours and Emergency Care
If you have a medical or life-threatening emergency, please call 911. For any immediate or urgent needs, please call our office at 303.805.1807 and provide your name, contact
information, and clinical concerns to the answering service who will
page Dr. Jerath and deliver your message on your behalf. Unless, Dr.
Jerath is involved with a patient emergency, she will return your call
within 15-30 minutes. Please keep your phone line open, unblock your
phone line, and remain available to accept an incoming call from Dr. Jerath.
Prescriptions and Prescription Refills
Prescription refills may be requested by calling our office directly, leaving a message on our prescription renewal line, having your pharmacy send a electronic refill request, or by submitting a request through our secure patient portal. Please allow at least 3 business days for prescriptions refills. We kindly ask that you provide us advance notice and plan accordingly for your prescription renewal.
Provided you have been seen within the past year with a current well woman exam or annual exam and we initially prescribed your medication, we can provide a prescription refill. If you need a refill on your oral contraceptive pills or hormone replacement therapy and are due for an annual exam, we will refill your prescription for one month as a courtesy until your appointment.
Prescriptions are only refilled during office hours and will not be addressed after hours or on the weekends. All prescription renewals are sent electronically to your pharmacy of record.
