If you are scheduled for a new patient appointment at our office, please click on our secure patient portal on the home page or my account link above to get started. Please follow the instructions to register and set-up your account. Please click on the health forms tab within the patient portal and fill out all 7 forms and remember to hit submit. This will take approximately 30-45 minutes. Please complete ALL forms 48 hours prior to your appointment or your appointment may need to be rescheduled. If you are interested in transferring your medical records to Optima Women's Healthcare please download the medical records release form by clicking on the appropriate link below. Once you have printed and completed the form, please fax to us at 303-595-5390 or email to us at staff@optimawomenshealthcare.com. Thank you. We look forward to caring for and serving you!
